Jobing Description

Staff Nurse/ Nurse Case Manager to work in conjunction with and under the supervision of prescribing psychiatrists, physician assistants, and nurse practitioners to provide case management services which are directly related to medications which patients receive from MHP. The Nurse Case Manager will be first line response for patient phone calls or drop-in visits related to medication questions, e.g. refills, side effects, distributing medication, dosage and administration, medication interactions, symptoms that may require a change in treatment. May do routine medical tests, e.g. blood draws, blood pressure, blood sugars. May include some direct patient contact, e.g. medication groups, routine follow-up appointments, checking side effects, etc. Role is intended to support both efficient medical practice and good customer service to consumers. Provides limited crisis intervention by telephone or face-to-face, primarily for medication-related concerns. May have direct consumer contact, e.g. in medication groups, routine follow-up visits, checking side effects, administering medications including shots, doing routine blood draws, blood pressure, weights, blood sugars. May participate as a co-therapist in medication stabilization and maintenance groups under the supervision of a psychiatrist or mid-level practitioner.

Skills / Requirements

R.N. licensed to practice in Colorado with six years relevant experience. R.N. with relevant B.S.N. M.A. or M.S.N. in Psychiatric Nursing and three years relevant experience. Relevant experience means working in an inpatient or outpatient setting with severely and persistently mentally ill clients. Some outpatient experience preferred. Must have excellent knowledge and skills in current psycho-pharmacology, psychiatric diagnosis, and psychiatric intervention. Excellent customer service skills, including excellent telephone skills. Basic computer skills required and must be able to sufficiently use electronic medical record, Microsoft Word, e-mail and ability to learn additional tools, e.g. Excel. Bilingual, Spanish speaking preferred, but not required. Must have the commitment and flexibility to ensure excellent customer service and teamwork in a fast-paced environment. Background check and credentialing required. MHP application required. Must have reliable vehicle, a current CO driver‚s license. Minorities strongly encouraged to apply.
